名古屋大学の脊椎班は、脊柱変形、脊椎・脊髄腫瘍、靱帯骨化症など一般的に難治性といわれる疾患に加えて、椎間板ヘルニア、腰部脊柱管狭窄症、頚髄症といった頻度の高い脊椎疾患に対しても治療を行っています。毎週木曜日に脊椎脊髄専門外来を、火曜日と木曜日に手術治療を行っております。
このような診療に加えて、若手を対象とした脊椎セミナー、全国の脊椎脊髄専門医を対象とした頚椎セミナー、関連病院の医師が集まるケースカンファレンスも定期的に行い、教育研修の機会を多く設けております。
また研究面においては、靱帯骨化症の治療検証、脊椎腫瘍の解析、術中脊髄モニタリングなどの研究に加え、脊髄再生、骨再生など臨床応用につながる様々な研究を行っております。

診療概要

診療方針

脊柱変形(側弯症)、腫瘍(脊椎、脊髄)、靱帯骨化症(とくに胸椎)という脊椎疾患の中でも難治性といわれる疾患を主に加療しております。難治性疾患は、比較的頻度が少なく、その病態解明が十分でないこと、治療法が未確立であることが多いです。当施設ではこれらの疾患に対し、臨床、基礎研究の両面から解明していくことで、成績の良い(術後麻痺のない)治療法の確立を目指しております。また、非常に稀な疾患の患者様に対しては、国内外で経験のある医師との情報共有をはかり、最先端の医療を提供できる体制をとっております。

以下に当施設で治療している主な疾患の説明をさせていただきます。

特徴的な治療

脊柱変形(側弯症)

脊柱変形は様々な原因により脊柱が三次元的にねじれてくる状態を指します。側弯症が進行すると、立位バランスの悪化や腰背部痛、心肺機能の低下をきたすことがあります。脊柱側弯症は小児から思春期のものと成人期のものに大別され、小児から思春期の側弯症(図1)には、特発性側弯症、先天性側弯症、症候群性側弯症などが含まれます。成人期に生じる脊柱変形(変性側弯)(図2)は加齢性の変性変化が脊柱に加わり、脊髄の入る脊柱管の狭窄を伴う症例も多いため、腰背部痛や脚のしびれが出現して日常生活にも支障を来すことが多くなります。
当院では安全に手術を行うために、術中CTナビゲーションシステムを用いて手術を行っております。また術中脊髄モニタリングを行い、常に脊髄の状態を把握することで、側弯矯正の際に生じる可能性のある脊髄損傷を未然に防ぐ様にしております。これらにより高度な変形に対する骨切り術、3次元的矯正などの手術も、より安全かつ正確に手術を行うことが可能です。また成人期に生じる脊柱変形に対しては、側方経路椎体間固定術などの低侵襲手術を行うことで、高齢者にも安全に手術をうけて頂くことが可能です。

靱帯骨化症

靱帯骨化症は国の指定する難治性疾患ですが、比較的頻度の高い頚椎靱帯骨化症に加え、難治性とされる胸椎縦靱帯骨化症に対する手術治療にも積極的に取り組んでおります。胸椎靱帯骨化症に対する手術は、術中に脊髄障害(麻痺)の出現する確率が高く、麻痺発生率が約30%(全国調査)と報告されているため、いかに安全に手術を行い、術前の重度の脊髄障害を回復させるかが重要となります。当院では頚椎後縦靱帯骨化症に対しては、後方から脊柱管を拡大する椎弓形成術、金属を併用した後方除圧固定術に加えて、前方除圧固定術を症例に応じて適切に選択しております。胸椎レベルの靱帯骨化症に対しては、後方からの除圧のみでは麻痺発生の危険性が高くなるため、金属を併用した後方除圧矯正固定術を行っております。後方から矯正固定術を行うことで、胸椎の後弯(後ろへのカーブ)を減少させて、間接的な脊髄への除圧も可能となります。後方からの手術後に脊髄障害の回復が思わしくない症例では、後方から脊髄前方に位置する骨化巣を切除することもあります。現在、厚生労働省「脊柱靭帯骨化症に関する調査研究」における胸椎靭帯骨化症手術前向き研究を、当大学病院を中心に行っており、この結果がさらなる治療成績向上の一助となることを期待されております。

脊椎腫瘍

脊椎腫瘍の多くは転移性腫瘍(癌の転移)によるものが多く、原発性脊椎腫瘍は比較的稀とされています。当院では難治性の原発性脊椎腫瘍に対しても積極的に治療をしております。良性腫瘍(血管腫、類骨骨腫、骨芽細胞腫、骨軟骨腫、Pigmented villonodular synovitis(PVNS)、線維腫、線維性骨異形成)に対しては、腫瘍を摘出して、必要に応じて固定術を行います。中間型といわれる巨細胞腫は、以前は手術を行っても再発率の高い腫瘍でしたが、破骨細胞の機能を抑える抗RANKL抗体であるデノスマブ(商品名ランマーク)を投与して腫瘍の縮小を待ってから手術を行うことで、腫瘍の再発率を抑えることができるようになりました。
悪性腫瘍(軟骨肉腫、骨肉腫、MFH、脊索腫、Ewing肉腫、胞巣状軟部肉腫、類上皮血管内皮腫、血管肉腫、線維肉腫)では、手術加療に加えて、化学治療、放射線治療、粒子線治療(陽子線、重粒子線)を併用した集学的治療を行っております。

脊髄腫瘍

脊髄実質内に発生する比較的稀な腫瘍で、首・背中・腰の痛み、腕や脚の運動麻痺や感覚障害(しびれ)、歩きにくさ、膀胱直腸障害などが出現します。腫瘍またはその周辺から出血が起こり、症状が急激に悪化することがあります。組織学的には、上衣腫、神経膠腫が多くみられ、血管芽腫、脂肪腫、髄内神経鞘腫、海綿状血管腫、脊髄内転移性腫瘍などの腫瘍も認められます。良性腫瘍では手術で全摘出を目指しますが、悪性度の高い腫瘍では化学療法が必要になることもあります。また手術所見だけではなく、術中病理診断も行い、腫瘍の摘出範囲などを適切に判断しています。

術中脊髄モニタリング

脊椎手術を安全に行うために、術中脊髄モニタリングを使用しております。手術中に起こる可能性がある脊髄神経障害をモニターするために、手足および頭の表面に電極をつけて運動野を刺激します。神経の活動は電気の波形となってコンピューターのモニター上に記録され、それを指標に手術を行います。脊椎の手術中は全身麻酔で患者様は眠っているため、このモニタリングシステムでの波形を観察することで、神経障害の可能性をリアルタイムに同定します。手術操作により波形の大きさが著しく変化した場合は、操作を中断するなどし、脊髄の機能を守ります。

O-armナビゲーションシステム(移動型デジタル式汎用一体型X線透視診断装置)

O-armは手術中に撮影した精度の高いCTを撮影することができ、ナビゲーションシステムと連動することで、手術操作を補助します。頚椎や側弯症の手術でスクリューを骨内に挿入する際は、神経、血管などの重要臓器への誤刺入が問題となりますが、本ナビゲーションシステムを用いることで、高精度なスクリュー刺入が可能となります。スクリュー挿入の際のナビゲーションのみでなく、挿入されたスクリューをO-armで確認できるため、精度の高い、安全な手術が可能となります。さらに脊椎腫瘍に対しても、腫瘍の取り残しがないか確認が可能であり、腫瘍の再発率減少にもつなげております。

研究概要

臨床研究成果

当科で手術を行う脊椎脊髄腫瘍、脊柱変形、脊椎靱帯骨化症は、術後の麻痺を含めリスクが高く難治性疾患といえます。疾患の病態解明や患者さんの症状をより改善する手術法、適切な手術機器の使用など、国際的にも解決できていない課題に対し、名古屋大学内での臨床・基礎研究に加え、30病院以上の名古屋大学関連施設(Nagoya Spine Group)の手術データ(脊椎手術約73,800例:2020年6月現在)を解析し、患者さんにとって、より良い手術方法・手術タイミングなどを国内、海外に報告・発信して参りました。毎年3月には一般市民向けの市民公開講座も行っております。このような研究や活動を通じて未だに限界のある医療の原因を解明し医療を発展させることが、患者さんの治療に貢献する当院の責務と考えています。

脊髄損傷における神経再生

現在の日本では約10万人の以上の脊髄損傷患者が麻痺を抱えたまま生活を余儀なくされており、毎年5000人以上の患者が脊髄損傷を来たしております。近年は若い世代だけではなく、軽微な転倒で発症する高齢者の非骨傷性頚髄損傷も増加しており、大きな社会的問題となっております。脊髄神経はいちど障害が生じると十分な回復が得られず、脊髄損傷に対する効果的な治療法は確立されていません。名古屋大学整形外科脊椎グループは分子生物学・門松健治教授のグループ、神経遺伝情報学・大野欽司教授のグループと協力し、脊髄損傷に対する基礎研究を行っています。門松研究室では新しい神経軸索再生抑制因子と予測されるプロテオグリカンの1種であるケラタン硫酸の神経再生効果に関する研究を、大野研究室では神経筋接合部のタンパクレベルおよびRNAレベルにおける分子病態機構の解明や、既存薬のオフラベル薬効の臨床応用に向けた研究をしています。

共同研究

八雲研究

もともと八雲町は明治初期に徳川藩が入植活動をしてできた町です。その為今でも名古屋とゆかりのある町です。1980年代に八雲町が住民検診を始めようとした際に、名古屋大学に依頼があり、ボランティアとして始めた事がきっかけとなり、以降毎年住民検診を行いながら疫学調査を行っています。整形外科に限らず、検査科、耳鼻科、眼科、泌尿器、心理学などなど多岐にわたる調査をいろいろな大学と合同で実施しています。

脊髄再生、骨再生に関わるペプチドゲルの有用性

近年iPS、ES細胞などの幹細胞を使用した再生治療が注目されるなか、その足場となる媒体の使用については未だ一定の見解が得られていません。当研究室では企業との共同研究により、神経再生、骨再生に対する新規自己集合体ペプチドゲルの効果を、マウス脊髄損傷モデル、ラット大腿骨欠損モデルを使用して調査しております。自己集合体ペプチドは、pHは髄液と同様の中性で細胞毒性はなく、十分な強度、細胞接着性をもつ(Nagai Y et al. Biomaterials33: 1044-51, 2012)。現在のところ、脊髄前角細胞より採取した初代神経細胞培養において神経軸索突起が有意な伸長効果、神経栄養因子およびその受容体の発現上昇を確認できました。またin vivoにおいてはこれらの因子に加え神経軸索再生に関与しているMMP-2, 9の発現上昇と脊髄損傷後の神経再生阻害に関与するグリア性瘢痕の減少を認めました。これは一度損傷が生じても、細胞が発育するに快適な環境が存在することで得られた結果と判断できます。骨再生についてはin vitroにおいて骨芽細胞のcell-lineであるMC3T-E1を使用した培養において骨形成のマーカーであるosteocalcinとosteopontin、そしてALPの発現上昇を確認でき、in vivoにおいてはラット大腿骨欠損モデルを作成したところ、コントロール群に比較し、CT画像上有意に骨形成が促進していました。骨芽細胞の活性化に関わるペプチドゲルを、高齢化社会に伴い増加している骨粗鬆症を伴う脊椎手術の際に臨床応用することで、手術成績向上に直結し、今後問題となるであろう医療費増加に対する解決の一助ともなることが大いに期待されます。
